PSMA PET/CT: A Clearer Way to See Prostate Cancer

Hearing the words “prostate cancer” can be overwhelming. Along with the physical challenges, there are also difficult decisions about which tests and treatments are right for you. One of the most important advances in recent years is a scan called PSMA PET/CT, which allows doctors to see prostate cancer with far greater accuracy than standard imaging.

Understanding the Prostate

The prostate is a small gland in the male reproductive system, found just below the bladder and in front of the rectum. Its role is to make seminal fluid, which mixes with sperm from the testicles. As men age, the prostate often changes, sometimes leading to issues such as enlargement, inflammation, or cancer.

Routine blood work, like a PSA (prostate-specific antigen) test, can alert your doctor when something is abnormal. But a high PSA level alone cannot tell whether cancer is present or where it may have spread. That’s where advanced imaging like PSMA PET/CT becomes important.

What Makes PSMA PET/CT Different?

PSMA PET/CT combines two powerful tools:

  • PET (Positron Emission Tomography): Shows areas of high activity in the body by detecting a small amount of radioactive tracer.
  • CT (Computed Tomography): Provides detailed pictures of bones and soft tissues.

The “PSMA” part refers to prostate-specific membrane antigen, a protein that is abundant on most prostate cancer cells. When the tracer attaches to PSMA, cancer cells become visible on the PET scan. When those images are combined with CT, doctors get a precise map of where the cancer is located

Why It Matters

PSMA PET/CT is changing how prostate cancer is diagnosed and treated because it can:

  • Detect hidden spread: Pinpoints cancer in lymph nodes, bones, or organs that may not appear on traditional scans.
  • Identify recurrence: Finds cancer that has returned after surgery, radiation, or hormone therapy.
  • Guide treatment decisions: Helps doctors select the right therapy, whether surgery, radiation, hormone therapy, or new targeted options.
  • Personalize care: Provides a clear picture so treatment can be delivered exactly where it is needed.

What to Expect During the Scan

  1. Tracer injection: A small amount of a safe radioactive tracer is given through a vein in your arm.
  2. Resting period: You will wait quietly for 30 to 60 minutes while the tracer circulates through your body.
  3. The scan itself: You’ll lie on a cushioned table that slowly moves through the PET/CT machine. The scan is painless and usually lasts 20 to 30 minutes.

Most patients can return to normal activities right away. Your care team will let you know if any special preparation is required, though in many cases you can eat and take your usual medications as normal.

After the Test

A radiologist reviews the images and shares results with your doctor. Together, you and your doctor will discuss what the findings mean and whether additional treatment or monitoring is needed. In some cases, a PSMA PET/CT scan can replace multiple other tests, saving valuable time in making decisions about your care.
